For claims outside the US and Canada, please visit your local SMITH Optics retailer.

Accepted files include a pay stub from within the last 3 weeks (feel free to erase sensitive information like account numbers), PSIA card, Employee ID, current guide license, etc. Please note that business cards, driver’s license, photographs, old paystubs or any expired documents can’t be used and need not be uploaded.
